After two months Matthew Perry he died at the age of 54 in October 2023, his death was ruled an accident. That decision seemed far from the end of the story.
A December 2023 toxicology report concluded that Perry died of “the side effects of ketamine.” The report noted that Perry had been receiving regular ketamine infusions to treat depression and anxiety, but his last infusion occurred more than a week before his death. The discrepancy left room for questions about why Perry had ketamine in his system when he died.
In May 2024, the Los Angeles Police Department confirmed that they had opened an investigation into Perry’s death. Three months later, five people – Dr. Salvador Plasencia, Jasveen Sangha, Kenneth Iwamasa, Erik Fleming again Dr. Mark Chavez – were charged with various charges, including conspiracy to distribute lethal ketamine.

September 30, 2023
According to court documents obtained by Us Weekly in August 2024, “after learning” that Perry was “interested in obtaining ketamine,” Plasencia allegedly contacted Chavez about purchasing the drug “so he could sell ketamine” to the actor. The doctors also said that Plasencia wrote a message to Chavez saying, “I wonder how much this fool will pay” for this.
After Chavez sold Plasencia, a licensed physician, “at least four vials of liquid ketamine, ketamine lozenges,” syringes and gloves, documents allege that Plasencia left the drug with Perry’s former assistant Iwamasa and taught him how to inject ketamine so he could. directed at Perry.
October 2, 2023
Iwamasa sent Plasencia a text message asking to buy more vials of ketamine.
That same day, Placencia texted Chavez asking for more ketamine to sell to Perry.
October 8-10, 2024
Plasencia sold several vials of ketamine to Iwamasa, and met both him and Perry in several locations, including Long Beach, California, where Plasencia allegedly injected Perry with the drug while in a car.
The documents also state that on October 10, Iwamasa contacted Fleming to purchase more containers of ketamine. Fleming then sent Sangha, also known as “The Ketamine Queen,” to get the drugs.
October 14, 2023
After delivering ketamine samples to Iwamasa and Perry at their residence the day before, Fleming allegedly drove to Perry’s home to collect money for the drugs. Fleming then drove to the “Sangha’s house” and bought 25 bowls. He then drove back to Perry’s house and delivered the drugs, according to the documents.
October 23, 2023
Iwamasa texted Fleming asking to buy more ketamine, allegedly asking in writing, “Can we do the same for the last time in the next two days?”
Fleming then went to Perry’s house to collect cash from Iwamasa.
October 24, 2023
Fleming allegedly bought another 25 vials of ketamine from Sangha, then returned to Perry’s home to drop off the drugs.
October 25-27
Court doctors alleged that Iwamasa injected Perry with “at least” six injections of ketamine on October 25, 26 and October 27.
October 28, 2023
Perry was found unresponsive in a hot tub at her home in Los Angeles. He was pronounced dead later that day.
In documents obtained by Usit was alleged that Iwamasa injected Perry with “multiple doses of ketamine he received” from Fleming and Sangha, “resulting in the death and grievous bodily harm” of the late actor. The doctors also said that Placencia gave the injections.
That same day, Sangha ordered Fleming to “dissociate himself from selling ketamine” to Perry, including deleting digital evidence from their cellphones.
October 30, 2023
Fleming is said to have written a message to Sangha asking how long ketamine stays in someone’s system since a “three month detox” will be done on Perry.
December 2023
The Los Angeles County Medical Examiner’s office released a toxicology report concluding that Perry died of “the side effects of ketamine.” The actor’s death was determined as accidental, with drowning, coronary artery disease and the effects of buprenorphine (used to treat opioid use) listed as contributing factors.
May 2024
The Los Angeles Police Department confirmed that they have opened an investigation into Perry’s death, focusing mainly on how Perry got the ketamine in his system when he died.
August 2024
The U.S. Attorney’s Office for the Central District of California shared a press release announcing that Dr. Salvador Plasencialicensed physician, and Jasveen Sanghaknown as “The Ketamine Queen,” an alleged drug dealer, was arrested on August 15 in connection with Perry’s death. Both Placencia and Sangha were charged with one count of conspiracy to distribute ketamine.
Sangha was also charged with one count of maintaining a drug facility, possession with intent to distribute methamphetamine, possession with intent to distribute ketamine and five counts of possession of ketamine; while Placencia is also charged with seven counts of distribution of ketamine and two counts of altering and falsifying documents or records related to a government investigation.
The press release also named three other people who were charged separately in Perry’s death: Iwamasa, an acquaintance of Fleming, and Dr. Chavez, another doctor.
Fleming pleaded guilty on August 8 to one count of conspiracy to distribute ketamine and one count of distribution of ketamine resulting in death. According to the U.S. Attorney’s Office for the Central District of California, Fleming admitted in court papers that he distributed the ketamine that killed Perry.
Iwamasa pleaded guilty on August 7 to one count of conspiracy to distribute ketamine causing death, and admitted to repeatedly injecting Perry with ketamine without medical training, including on the day he died. Chavez pleaded guilty to one count of conspiracy to distribute ketamine.
United States Attorney Martin Estrada in the statement we said: “These defendants were very concerned about benefiting from Mr. Perry rather than caring for his well-being. Drug dealers who sell dangerous substances risk other people’s lives out of greed. This case, along with many other prosecutions of drug traffickers who cause death, sends a clear message that we will punish drug traffickers for their deaths.”
